For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    a)  Jobsite located at 648 Savannah Road/Scaffolding - On or about, and at times prior to  August 21, 2012, the employer did not ensure toe boards or other forms of falling debris protection was provided to protect workers below, thus exposing employees to struck-by hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.      a)  Jobsite located at 648 Savannah Road - On or about August 21, 2012, employees were using cross bracing and scaffold up-rights to ascend and descend the first and second level scaffold system, thus exposing employees to fall hazards up to 13 feet.
