29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a. Mezzanine 201: There was a walking working area of approximately 22 feet 5 inches wide where the perimeter top guard rail consisting of a 3/8 inch diameter cable varied in height from 36 1/2 inches to 37 1/2 inches and could be pushed down to 31 inches from the floor which was not adequate to protect employees from a fall of approximately 15 feet to the lower level below as observed on 8-22-12.
