29 CFR 1910.303(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed. On August 21, 2012 and times prior to, employes were exposed to openings that were on the electricla pannel suppling power to the contractor's trailer. This conditon exposed employees to electrical shock.

29 CFR 1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear. On August 21,2012, and times prior to, a conductor entering an electrical panel was not provided with bushing. This condition exposed employee to electrical shock.
