29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables were used:    a) GITO, Inc. dba Nello Construction / 220 Scotland Road, Edinboro, PA: On or about August 22, 2012 the outer jacket at the plug end of an extension cord was not retained in place with the cable grip and exposed the electrical wires underneath. The extension cord was powering a DeWalt circular saw being used on the first floor by the addition.

29 CFR 1926.405(a)(2)(ii)(I):     Protection was not provided to avoid damage when flexible cords and cables passed through pinch points:     (a) GITO, Inc. dba Nello Construction / 220 Scotland Road, Edinboro, PA: On or about August 22, 2012 in multiple areas of the jobsite the drywall was installed around the flexible cords used for temporary electrical wiring.  Protective devices were not installed in and/or around the area where the cord passed through the wall.  The areas included, but are not limited to, the 1st floor by the addition, lower level South West part of the building in a storage room or conference room, North West section of the atrium room on the right side by hall, near and in room b29A, near and in room b33A, near and in b35A.
