On or about August 23, 2012, at the bottom of the stem positioner rotor chamber, maintenance employees standing on the I-beams were exposed to a 7 feet fall hazard.

On or about August 23, 2012, the lockout tagout procedures for the stem positioner rotor were not adequate in that it did not account for the accumulation of stored energy, exposing employees to the hazard of being struck by the blades.
