29 CFR 1926.451(b)(8):     At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over the same bearer was not laid second, on top of the first platform:    a.  On or about August 21, 2012, the scaffold platform had a gap at the corner of the platform, exposing employees to a 14 feet fall hazard.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:    a.  On or about August 21, 2012, the scaffold platform did not have a ladder or other safe means of access, exposing employees to a 14 feet fall hazard.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:      a.  On or about August 21, 2012, the scaffold platform did not have a guardrail in some sections and it did not have a mid-rail in other sections, exposing employees to a 14 feet fall hazard.
