29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    a) Residential jobsite: On August 10, 2012 and at times prior thereto employees performed stucco activities from a welded frame scaffold that was set up without mud sills in place.    Note) River Front Exteriors, Inc., was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA inspection 313357246, citation 1, item 1b, issued on January 7, 2011 and was affirmed a final order on February 5, 2011, with respect to the workplace located at I-15 and Sunnyside, Idaho Falls, 83402.         Note) Abatement certification and supporting documentation ARE required for tis item.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:     a) Residential jobsite: On August 10, 2012 and at times prior thereto employees performing stucco activities climbed the scaffold frame for access to the upperlevels.      Note) River Front Exteriors, Inc., was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA inspection 313357246, citation 1, item 1c, issued on January 7, 2011 and was affirmed a final order on February 5, 2011, with respect to the workplace located at I-15 and Sunnyside, Idaho Falls, 83402. 29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    a) Residential jobsite: On August 10, 2012 and at times prior thereto employees performed stucco activities from a three tiered scaffold system without a fall protection system in place.  Employees were exposed to fall hazards up to 20 feet from the top level to the ground below.
