29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about August 24, 2012, during roofing activities, the employer allowed employees to work on a roof that was approximately 19 feet at its highest eve and 10 feet at the lowest eve.  The roof had a combination of 10/12 and 4/12 pitchs.  The employer did not ensure that the employees were protected from falls

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On or before August 24, 2012, during roofing activities, the employer allowed employees to use an extension ladder that was not extended 3 feet above the landing or secured to prevent employees from falling while accessing the ladder or the roof.
