29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork: Note: Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift. a) At the jobsite, on or about 8/17/12: Two employees were using a work platform supported by the forks of a rough terrain forklift. The work platform was not securely attached to the forks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) At the jobsite, on or about 8/16/12: Employees were exposed to falls of 17 feet 6 inches while working from a platform supported by the forks of a rough terrain forklift. b) At the jobsite, on or about 8/16/12: An employee working on the front facade of the building was not protected from falling more than 17 feet to the ground below.
