29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: The employer did not ensure accessability to the electrical pannel located in the far left corner in the office supply storage room and near employee bathroom as it was blocked with material.

29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops shall have ground-fault circuit-interrupter protection for personnel. Employer did not ensure that employee bathroom was protected with ground fault circuit interrupter protection (GFCI). This was observed on 24 August 2012 in the employee restroom located on the northeast side of building that had a receptacle within 3 ft of a sink that was not protected by GFCI.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Strain relief was not provided on the cord to the shredder. The insullation had seperated from the plug and exposing the conductor. This was observed on or about 24 August 2012.
