Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) GL Quality Contractors LLC at 16442 E 96th Way, Commerce City, CO 80022: On or before August 25, 2012, the employer did not ensure that the grounding path for the Woods Splitter, Serial Number E204300388, was continuous. The splitter was used to power a 110 volt circular saw. Condition exposed employees to electrical shock hazards.
