29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On or about August 22, 2012, and times prior thereto, at the 230 Bull St. jobsite, the employer failed to assure strain relief to a flexible extension cord on the floor at the parlor-level office entrance, with outer insulation pulled back from the male end, exposing employees to electrical hazards not limited to overheating, fire and/or electric shock.
