29 CFR 1926.760(c)(3): The boundaries of a CDZ were not designated and clearly marked. The CDZ shall not be more than 90 feet (27.4 m) wide and 90 (27.4 m) feet deep from any leading edge. The CDZ shall be marked by the use of control lines or the equivalent. Examples of acceptable procedures for demarcating CDZ's can be found in Appendix D to this subpart: a) Brandywine Senior Living at Voorhees, 311 Route 73, Voorhees, NJ 08043: On or about August 21, 2012, employees were exposed to fall hazards while installing new decking in a CDZ that was not designated and clearly marked.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) Brandywine Senior Living at Voorhees, 311 Route 73, Voorhees, NJ 08043: On or about August 21, 2012, employees were observed exposed to falls of approximately 23 feet while walking the second level horizontal I-beams. GENERAL FABRICATORS & WELDING INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1926.760(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311564777, CITATION 1, ITEM 1, ISSUED ON 3/25/2008, FINAL ORDER DATE 12/22/2011.
