29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a)  Jobsite at 2500-2502 Broad Avenue, Altoona, PA:  On or about September 24, 2012, an employee was working from a tubular welded frame scaffold and the working level was  not fully planked.  The fall hazard to the employee was approxiamtely 18 feet.

29 CFR 1926.451(b)(10):  Scaffold components manufactured by different manufacturers shall not be intermixed unless the components fit together without force and the scaffold's structural integrity is maintained by the user. Scaffold components manufactured by different manufacturers shall not be modified in order to intermix them unless a competent person determines the resulting scaffold is structurally sound.    a) Jobsite at 2500-2502 Broad Avenue, Altoona, PA:  On or about September 24, 2012, two employees were working from different scaffold components that were used to make one scaffold.  A tubular welded frame scaffold and a ladder jack scaffold were used to form a scaffold.  The two employees were working from this combination of scaffolding types.  The fall hazard to the two employees was approximately 18 feet to the ground.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above a lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite at 2500-2502 Broad Avenue, Altoona, PA:  On or about September 24, 2012, employees were painting and performing exterior work from  scaffolding that did not have any guardrails on it, nor was any fall protection being utilized.  The fall hazard to the employees was approximately 18-20 feet to the ground below.

29 CFR 1926.454(b):  The employer did not have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicablicable:      *The nature of scaffold hazards.    *The correct procedures for erecting, disassembling, moving, repairing, inspecting, and maintaining the type of scaffolds  in question.    *The design criteris, mazimum intended load-carrying capacity and intended use of the scaffold.    *Any other pertinent requirements of this subpart.        a) Jobsite at 2500-2502 Broad Avenue, Altoona, PA:  On or about September 24, 2012, the employer did not have each employee who erected, disassembled, maintained, inspected, repair or move a staffold trained by a competent person to recognize the hazards associated with this type of scaffold work.
