29 CFR 1910.119(h)(3)(i): The contract employer failed to assure that each contract employee was trained in the work practices necessary to safely perform his/her job:    21178 SW 89th Rd., Beatrice, NE 68310 (Jobsite): There was no documentation or other evidence that each subcontractor employee (Furmanite) was trained to monitor / check for atmospheric pressure in the High Temperature Shift vessel before removing a mechanical plug.  The contract employer (Cust-O-Fab) did not assure that each subcontractor had been so trained.  On August 17, 2012, one (1) contract employee was injured and one (1) subcontract employee was hospitalized after a mechanical plug, which was not designed for holding pressure behind it, suddenly came out as the subcontract employee was releasing the bolts.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
