29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program (Lockout/Tagout) was understood by employees, and to ensure that employees acquired the necessary knowledge and skills required for the safe application, usage and removal of energy control devices: (a) Atlas Metal & Iron Corp. @ 1100 Umatilla Street, Denver, CO. On or before August 27, 2012, the employer did not ensure that each employee involved with performing maintenance was provided adequate training on the functions of the Written Equipment Specific Lock-Out/Tag-Out Energy Control Procedures to ensure that each employee acquired the necessary knowledge and skills required to follow the safe procedures, usage and removal of energy control devices when performing servicing or maintenance operations on machines and equipment, including but not limited to the following: (1) Cranes: Hydraulic, Rubber Tire & Track, Cable Machines; (2 )Chopping Blade Grinder (3) MTB Shredder Machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedures and requirements were followed as required under the standard: (a) Atlas Metal & Iron Corp. @ 1100 Umatilla Street, Denver, CO. On or before August 27, 2012, employer did not conduct an annual or more frequent inspections of the Written Equipment Specific Lock-Out/Tag-Out energy control procedures to assure those procedures were implemented and followed properly by the employees and were familiar with their responsibilities under those procedures while performing servicing or maintenance operations on machines and equipment, including but not limited to the following: (1) Cranes: Hydraulic, Rubber Tire & Track, Cable Machines; (2 )Chopping Blade Grinder (3) MTB Shredder Machine.
