29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c).     The employer does not protect employees from cave-ins while working in excavations 5 feet or more in depth.  This violation occurred on or about August 24, 2012, at the job site where employees were exposed to a crushed-by hazard while installing a natural gas pipeline in an excavation that was not protected from cave-ins.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from cave-ins when working in excavations.
