29 CFR 1926.25(a): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement. On or about August 27, 2012, during construction on the TC Jester overpass, employees were exposed to potential hazards when working above form and scrap lumber and other debris. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from impalement when working around protruding reinforcing steel.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. The employer does not protect employees from falling when working 6 feet or more above lower levels. This violation was observed on or about August 27, 2012, on the TC Jester overpass where employees were exposed to a 19.5 foot fall hazard when setting copings without fall protection. Williams Brothers Construction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 312126071, citation number 1, item number 5, and was affirmed as a final order on 6-28-10 with respect to a workplace located at the Margaret Hunt Hill Bridge, Dallas, TX 75207. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 6 feet or more above a lower level of ground.
