29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs (m) and (o) of this section: a) On or about August 28, 2012, at the above located workplace employees were required to work with and around a conveyer drive pully and belt which did not have a guard affixed around the pulley. The employees were thereby exposed to the hazards associated with unguarded rotating parts. .
