29 CFR 1926.100(a): Employees were working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, were not protected by protective helmets. On or about August 28, 2012, at Paco's Chicken and Beef business located at 6951 North Mesa, El Paso TX. an employee working at ground level was raising roof materials such as but not limited to plywood panels, tools, etc. to co-workers on a sloped roof with an elevation from ground of approximately 9 feet without any head protection. Employee is exposed to overhead stuck-by hazards.

On or about August 28, 2012, four employees engaged in low-slope roofing activities with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, were not properly protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. WICKES CONTRUCTION INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 405542, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON 05/24/12, WITH RESPECT TO A WORKPLACE LOCATED AT 7000 GATEWAY BLVD. EAST, EL PASO TX. 79915.

29 CFR 1926.502(d)(6)(v): Snaphooks were engaged to an object which were incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur. On or about August 28, 2012, at Paco's Chicken and Beef business located at 6951 North Mesa, El Paso TX. four employees engaged in low-slope roofing repair activities with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were lanyard connecting snaphooks and anchorage points were incompatible in shape or dimensioned in relation to the snaphook. This condition exposes employees to fall hazards due to an unintentional release or disengagement of the snaphook from the anchor point.
