29 CFR 1926.1053(b)(1): Portable ladder used to access the upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not be provided. At 9550 E. Lincoln, Wichita, KS, employee(s) were accessing the 5 in 12 sloped roof with a metal, extension ladder, which did not extend 3 feet above the eave and the ladder was not secure, exposing employees to an approximately 18 feet fall to the concrete and ground below.
