29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) On or about 27 August 2012 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to electrical (480 Volts) and hydraulics, for the 20 Arburg injection molding machines prior to the commencement of machine repair and maintenance operations. (b) On or about 27 August 2012 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to electrical (480 Volts) and hydraulics, for the 6 Engel injection molding machines prior to the commencement of machine repair and maintenance operations. (c) On or about 27 August 2012 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to electrical (480 Volts) and hydraulics, for the Sumitomo injection molding machine prior to the commencement of machine repair and maintenance operations. (d) On or about 27 August 2012 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to electrical (480 Volts) and hydraulics, for the Mitsubishi injection molding machine prior to the commencement of machine repair and maintenance operations.
