An employee who was required to use a Survivair valueair plus half mask dual strap respirator with Survivair multi-contaminant cartridge with P100 filters during lead pouring operations did not have his respirator in a clean and sanitary condition. A lead wipe sample taken inside the facepiece of the employee's respirator indicated concentrations of approximately 270 micrograms of lead.

The employee was exposed to an airborne concentration of formaldehyde which exceeded two parts formaldehyde per million parts of air (2 ppm) as a 15-minute STEL. The air monitoring conducted on September 4, 2012 indicated an exposure to formaldehyde of 2.67 parts per million (ppm) during one 15-minute sampling for a STEL.
