On or about April 26, 2012 - at the above addressed jobsite, the employer did not initiate and maintain an accident and prevention program to comply with the regulations of this part, which included but were not limited to, employee protection when exposed to falls.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On ro about April 26, 2012 - at the above addressed jobsite, employees conducting roofing operations from a roof that measured approximately twenty two (22) and twelve (12) feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
