29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)Production Area - A site specific lockout program was not developed. or implemented. Employees performed job tasks requiring lock out including, but not limited to changing the blade of the Spartan automated saw, and cleaning approximately sixteen CNC vertical milling machines, horizontal milling machines, and turning centers. On or about 8/28/12.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Production area - Approximately thirteen metal quad outlet receptacle boxes and nine metal two outlet receptacle boxes were being used as drop pendant outlets or extension cords. The receptacle boxes were not mounted to surfaces as required. On or about 8/28/12.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
