Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    (a)  On or about August 22, 2012, for the employees exposed to fire/explosion hazards where ignition sources, including but not limited to cigarette lighters and potential spark-producing tools, were not controlled when flammable vapors were ignited while servicing a crude oil storage tank at Cora 20-17 1H, 2TFH well site located at 32nd Street and 137th Avenue NW, Arnegard, North Dakota.
