29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: (a) On or about August 22, 2012, for the employees exposed to fire/explosion hazards where ignition sources, including but not limited to cigarette lighters and potential spark-producing tools, were not controlled when flammable vapors were ignited while servicing a crude oil storage tank at Cora 20-17 1H, 2TFH well site located at 32nd Street and 137th Avenue NW, Arnegard, North Dakota.
