29 CFR 1910.217(e)(1)(ii): Necessary maintenance or repair or both were not performed and completed on the clutch/brake mechanism, anti-repeat feature or single stroke mechanism of mechanical power press(es) before they were used: a) Main Plant: On August 16, 2012, and times prior thereto, employees were exposed to a stamping press with a defective flywheel brake that did not stop the flywheel after the light curtain was broken and the operator entered the machine operating area to make adjustments to the press. Note: Abatement certification is required for this item.
