29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On August 28, 2012 at a worksite located at 6S080 Lakewood, Naperville, IL., employees were performing work on a residential roof and were not using fall protection. Employees were exposed to a fall hazard of approximately 17.9 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: On August 28, 2012 at a worksite located at 6S080 Lakewood, Naperville, IL., employees were using a ladder that was placed on the first level to access the second level. The ladder was placed on the ridge of the roof and was not level and secure. Employees were exposed to a fall hazards while using the ladder. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
