29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    On or about August 29, 2012, employees performing roofing activities used portable hand tools powered by flexible extension cords that were worn, cut, and had pulled wires with exposed conductors.  Employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.      On or about August 29, 2012, employees working on a residential roof installing roofing tiles were not wearing personal fall protection nor protected from falling by other means of fall protective systems.  Employees were exposed to the hazard of falling.    a) On the top edge of the garage roof, an employee was exposed to the hazard of falling approximately 16 feet to the ground below.      b) On the top chimney roof, an employee was exposed to the hazard of falling approximately 28 feet to the ground below.    c) On the residential roof next to the chimney, an employee was assisting with tools and material and exposed to the hazard of falling approximately 12 feet to the ground below.
