On or about August 21, 2012, the electric motor connected to the pipe bender was wired to a toggle, or light, switch box intended for use in a fixed location.  The length of the cord allowed the 2-switch box to sit on the floor, enabling the operator to activate the switch with his foot.

29 CFR 1926.405(a)(2)(ii)(B):     Runs of open conductors were located where the conductors would be subject to physical damage and the open conductors were not fastened at intervals not exceeding 10 feet (3.05 m):    a)  Lower level, north side of the construction site at 809 French Street in Erie, PA.  On or about August 21, 2012, the Type NM Cables, used as temporary wiring in the hallway/work area, were wrapped around the temporary power station which caused the conductors to be excessively bent and twisted.  Additionally the loose conductors were hanging from the ceiling where they were subjected to damage from personnel/equipment moving through the area.
