OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to stuck-by and crushing hazards:     (a) Storms Specialty Services, Inc. @ 500 Swift Gulch Rd., Avon, CO 81620: On 8/28/12, the employer failed to ensure that a modular office unit was properly stabilized. Employees worked beneath the unit to remove the concrete masonry unit piers, which supported the approximately 14ft x 70ft modular office unit. The unit destabilized, fell off the piers and thereby partially crushed an employee.
