29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    a) Jobsite:  Springton Lake Middle School, N. Providence Road, Media, PA- Employees were installing a sheetmetal roof on a canopy approximately 15 feet above ground level without any means of fall protection.  Observed on or about 8/29/12.    THE WARKO GROUP WAS PREVIOUSLY CITED FOR A REPEAT VIOLATION OF THIS SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10)  WHICH WAS CONTAINED IN AN OSHA INSPECTION NUMBER 312978935 , CITATION NUMBER 1, ITEM NUMBER 1, WHICH WAS AFFIRMED AS A FINAL ORDER DATED  MARCH 7, 2011,WITH RESPECT TO A WORKPLACE  LOCATED  AT 400 HARRISON AVENUE, GLENSIDE, PA.    THE WARKO GROUP WAS PREVIOUSLY CITED FOR VIOLATIONS OF THIS SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10)  WHICH WAS CONTAINED IN AN OSHA INSPECTION NUMBER 312503683, CITATION NUMBER 2, ITEM NUMBER 1, WHICH WAS AFFIRMED AS A FINAL ORDER DATED 6/10/09, WITH RESPECT TO A WORKPLACE  LOCATED  AT N 12TH ST, READING CITADEL, READING, PA.    THE WARKO GROUP WAS PREVIOUSLY CITED FOR VIOLATIONS OF THIS SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10)  WHICH WAS CONTAINED IN AN OSHA INSPECTION NUMBER  311273650, CITATION NUMBER 1, ITEM NUMBER 1, WHICH WAS AFFIRMED AS A FINAL ORDER DATED 11/26/2008, WITH RESPECT TO A WORKPLACE  LOCATED  AT 5550 ROUTE 378, CENTER VALLEY, PA.    NOTE: Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
