On or about August 29, 2012, at Jared Store #436, 2150 Pleasant Hill NW, Duluth, Georgia:    An employee performing installing metal trim on the top of a round roof was exposed to a fall hazard of greater than 25-feet when the employee was not tied off to an anchor.     Duarte Roof & Gutters, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 312525256, citation number 1, item number 2 and was affirmed as a final order on May 22, 2009, with respect to a workplace located at Woodstock, Georgia.
