29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: a) Jobsite, 2240 McClellandtown Road, Masontown, PA: On or about August 20, 2012, during construction activities, the employer did not ensure that employees were protected from fall hazards of up to approximately 20 to 25 feet while on top of the mooring cells. A guardrail system existed on top of the mooring cells which had unused ladderway openings with a single chain stretched accross them at the top, but had no midrails.
