29 CFR 1910.253(b)(4)(iii):     Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    1) On or about August 29, 2012, the employer stored oxygen cylinders next to propane gas between the old foundry and new foundry buildings, without the required safe separation, or noncombustible barrier.

29 CFR 1910.1025(e)(1)(i):     Where any employee is exposed to lead above the permissible exposure limit for more than 30 days per year, the employer did not implement engineering and work practice controls (including administrative controls) to reduce and maintain employee exposure to lead in accordance with the implementation schedule in Table I below, except to the extent that the employer can demonstrate that such controls are not feasible.     1) On or aboout October 12, 2012 the employer did not implement engineering controls to reduce employees exposure to lead while working in the old foundry building.  Employees operating the furnace, which is used to produce bronze bars, were overexposed to lead while working in the old foundry building.

29 CFR 1910.1025(c)(1):     The employer did not assure that no employee was exposed to lead at concentrations greater than fifty micrograms per cubic meter of air (50 ug/m(3)) averaged over an 8-hour period.    1) On or about October 12, 2012 the employer did not ensure that employees were protected from airborne concentrations of lead while working in the old foundry building.  Employees operating the furnace, which is used to produce bronze bars, were exposed to airborne concentrations of lead at 0.12 mg/m3 as an 8-hour time-weighted average, which is 240% of the permissible exposure limit.
