29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: Well head area: On or about August 20, 2012, employees were operating a service rig during the reopening of a plugged oil well, one employee was not wearing protective eyewear. This exposed the employee to the hzard of being struck in the eyes by debris generated by the well rework.
