On or about August 27, 2012, KPL Lesley Lease, Loyal, OK, the employer did not maintain programs which ensured that a competent person would perform frequent inspections of the job site, materials and equipment during removal of a heater treater from a tank battery. Employees were exposed to the hazard of being struck by/crushed by falling equipment.
