29 CFR 1926.102(a)(2): Eye and face protection equipment required by this Part shall meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection. a) On August 29, 2012 and at times prior thereto, employees were not wearing safety glasses meeting the requirements of the American National Standards Institute standard Z 87.1-1968 while working with pneumatic nail guns and circular saws. Note: Abatement certification is required for this item.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a) On August 29, 2012 and at times prior thereto, three extension cords used throughout the work site had the cord pulled from the plug, exposing the conductors. b) On August 29, 2012 and at times prior thereto, the cord was pulled from the plug on the DeWalt DWS535 circular saw exposing the conductors. Note: Abatement certification is required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a) On August 29, 2012, the side rails of two extension ladders did not extend at least three feet above the roof eave on the front of the house. Note: Abatement certification is required for this item
