29 CFR 1910.24(f): "Stair treads." All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs. Yard area/south - On or about September 23, 2012, the rise height on the fixed stair was not uniform thruoghtout the flight of stairs. Employees were thereby exposed to trip and fall hazards.

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. Tank 2400 - On or about September 23, 2012, The employer did not effectively verify that all energy sources was isolated to the 2400 disconnect box. Thereby exposing employees to energized electrical parts.
