29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: a. Outside of Office Trailer - On or about August 31, 2012, the flight of stairs with more than four risers leading to the company's office was not equipped with a standard handrails on all open sides. Employees were thereby exposed to the hazards of fall.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: a. OutsideYard - On or about August 31, 2012, employees were required to remove rims from tires, batteries from cars, and engines from cars and the employer had not assessed the workplace to determine if hazards were present, or likely to be present which necessitated the use of personal protective equipment. Employees were thereby potentially exposed to eye and skin irritations.
