29 CFR 1910.133(a)(2):     The employer did not ensure that detachable side protectors (e.g. clip-on or slide-on side shields) met the pertinent requirements of this section:         a.  Carbon fiber processing & Bladder production - 08/30/12, slide-on side shields used by employees did not meet the pertinent requirements of this standard in that:         - The side shields did not provide adeqate protection from flying particles. The slide-on side shields did not fit firmly against the eyewear and face to protect the employee from eye hazards.         - The side shields were not certified to meet the pertinent requirements of ANSI Z87.1 - 1989 & 2003 which are incorporated by reference as outlined in paragraph (b)(1)(i) of this standard.

29 CFR 1910.303(g)(1):     Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment:    b.  Bay 2 - 08/29/12, sufficient access was not provided to the disconnect for the filter paper roll drive of th Tos Hostivar Grinder. The disconnect was located behind the machine.    c.  Carbon Fiber Processing - 08/30/12, sufficient access and required working space was not maintained around the disconnecting means for equipment in this room.  The ventilation duct from the lathe passed under the disconnecting means and a trash barrel was stored in front.

Near oxy-acetylene welding area - 08/29/12, the Graymills parts washer, which contained petroleum naptha, was not labled, tagged, or marked with the identity or information regarding the hazards of the contents.
