29 CFR 1926.404(b)(1)(iii)(C):     Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.    a. On the fourth floor of tower A, employees were using an extension cord with a missing ground prong, thereby being exposed to a contact hazard.    b. On the fourth floor of tower A in apartment 442, employees were using an extension cord with exposed interior wiring insulation at the plug, thereby being exposed to a contact hazard.    c. On the fourth floor of tower A in apartment 442, employees were using a job made power receptacle with exposed interior wiring insulation, thereby being exposed to a contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    a. On the fourth floor of tower A in apartment 442, employees were using an extension cord with exposed interior wiring insulation at the plug due to a failure of the strain relief, thereby being exposed to a contact hazard.    b. On the fourth floor of tower A in apartment 442, employees were using a job made power receptacle with exposed interior wiring insulation due to a failure of the strain relief clamp assembly, thereby being exposed to a contact hazard.

29 CFR 1926.502(b)(1):     Top edge height of top rails, or equivalent guardrail system members, shall be 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level. When conditions warrant, the height of the top edge may exceed the 45-inch height, provided the guardrail system meets all other criteria of this paragraph.  Note: When employees are using stilts, the top edge height of the top rail, or equivalent member, shall be increased an amount equal to the height of the stilts.    a. At the elevator shaft in the north/east corner of tower A, the top-rail guard rail was installed at approximately 47 inches, thereby exposing employees to a fall hazard.

29 CFR 1926.502(b)(2)(i):     Midrails, when used, shall be installed at a height midway between the top edge of the guardrail system and the walking/working level.    a. At the elevator shaft in the north/east corner of tower A, the mid rail guard rail was installed at 28.5 inches, thereby exposing employees to a fall hazard.
