29 CFR 1926.50(g): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. At the mixing area in the north/west corner of the jobsite, employees were mixing mortar containing Portland cement without a suitable eye wash station. b. On or near the scaffold where the employees were working with mortar containing Portland cement, a suitable eye wash station was not provided.

29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances. a. At the mortar mixing station, the employer failed to provide clean water, non-alkaline soap, and clean towels where employees are working with Portland cement.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On the east side of Tower A, employees were working on a scaffold side bracket 12 feet high without a guardrail on the south/east end, thereby being exposed to a fall hazard.

29 CFR 1926.452(c)(5)(iii): Brackets used to support cantilevered loads shall be used only to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold. a. On the south/east side of the facility, the mortar bucket was being placed on the exterior side brackets of the scaffold, thereby exposing employees to a fall or crushed by hazard.
