29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. Establishment-The employer had not certified in writing that he had evaluated the potential hazards and mandatory personal protective equipment associated with processes such as but not limited to changing feed hoses. An employee was injured when he was sprayed with chlorine from a hose.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury or illness had occurred. Establishment-The employee did not record an injury sustained by his employee for which the employee received medical care beyond first aid. The employee was injured by a chemical and was treated with an antibiotic that required a physician's prescription.
