OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Employees were exposed to overhead and crush hazards as the employer did not ensure lower hung cranes were installed and used per the ANSI/ASME B30.11-2004 requirements. The following deficiencies were found and did not meet the requirements:    1 - Initial inspection  Section 11-2.1.1- Employees were exposed to crush hazards as the employer did not ensure initial inspections were made to the 5 ton (serial #147132PF1) after it had been modified and installed at the workplace.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color.    On or about 8/30/12, in the metal fabrication area, where employees worked night shift, the employer did not ensure emergency exit signs were illuminated.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.179(j)(2): The items listed in subparagraphs (j)(2)(i)-(vii) were not inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, and all deficiencies were not carefully examined and a determination made as to whether they constituted a safety hazard.    On or about or about 8/30/12, employees were expoed to crush hazards as the employer did not ensure frequent crane inspections were made to cranes including the following cranes:    1 - 5 ton Kone Crane Serial #43097-8612 last annual inspection 2/15/2011 (480 volt) revealed required corrections were necessary hook latch damaged, spring broken, wire rope badly kinked, rope guide worn, follower rollers damaged, pendant station damaged, flat ribbon cables pulled out of junction box, Hoist low and high speed contactors burned out, minor wear on trolley pinion gear needs monitoring    2 - 2 ton Shaw Box Serial #32-93261 - last annual inspection 2/15/2011 (480 volt) revealed multiple broken lines in rope & needed replacement, too much rope on drum and needs shortening to proper length, pendant control cable pulled out of trolley J-box and is need of repair, Needed: replace trolley due to it wasnt made for hoist    3 - 3 ton Cleveland Tramrail Serial #Not Visible last annual inspection 7/15/2011 (460 volt) revealed capacity not labeled on both sides of bridge    4 - 3 ton Cleveland Tramrail Serial #EC3C.113.CW last annual inspection 7/15/2011 (460 volt) revealed capacity not labeled on both sides of bridge    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields.    On or about 8/30/12, in the metal fabrication area, employees were exposed to eye hazards as the employer does not require welding screens to be used in the metal fabrication areas to shield other employees working in close proximity to the area where welding is occurring.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
