OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by/caught between hazards from a roof truss collapse during installation:    (a)  DELETED    (b)  Garage/RV structure: On or about August 10, 2012, employees installed approximately 16 wood trusses spaning 25 feet without diagonal bracing.    (c) Garage/RV structure: On or about August 10, 2012, employees isntalled approximately 16 wood trusses spanning 25 feet without bottom cord horizontal bracing    Note 1) Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1):     Portable ladder(s) were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) were not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    (a) Back side of the garage structure: On or about August 10, 2012, and at times prior thereto, employees utilized two extension ladder(s) that did not extend to the level being accessed. 29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b).    a)  Job TU 19: On or about August 10, 2012, an employee was not protected from falling approximately 14 feet while walking/working on the top plate of the second story exterior wall.    Heavy Hammer Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(13), which was contained in OSHA inspection number 313353740, citation number 1, item 1, issued on May 21, 2010, and became final order on June 21, 2010, with respect to a workplace located on the corner of Downing & Wirth, Coeur d'Alene, Idaho.
