Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(b)  The Church of God Home, Incorporated - The employer provided only two of the three shots of the Hepatitis B vaccination series to one employee which was not in accordance with the current recommendations of the U.S. Public Health Service, on or about September 7, 2012.
