29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. This storage is not in accordance with the Compressed Gas Association, Inc. Pamphlet P-1 section 3.3.8 which states "Protect cylinders from any object that will produce a cut or other abrasion in the surface of the metal. Do not store cylinders near elevators or gangways, or in locations where heavy moving objects may strike or fall on them.": a) Heritage Firpo- The propane cylinders utilized as fuel for the fork truck were loose on a pallet between the two bay doors to the loading dock and exposed to damage. The area is the fork truck access to the loading dock and large shipping containers are handled within the area. Observed on August 31, 2012.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material: a) Heritage-Firpo- Wood Shop-The Craftsman Radial Arm Saw in the wood shop did not have a guard to protect the operator from the lower portion of the blade. Observed on August 31, 2012

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) Heritage Firpo- The employer did not certify that employees who operate the fork truck in the warehouse and loading dock were initially trained and evaluated in accordance with the requirements. The records provided during the inspection were limited to Certificates of Training for two employees for general classroom training from June 1999. Noted on September 18, 2012. Note: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a)Heritage-Firpo- Warehouse next to the door from the office area- Two electrical junction boxes on the power supply to the DSC MAXSYS fire Control/Communicator system were not provided covers. The systems primary is 120 volt AC. Observed on August 31, 2012 CORRECTED DURING INSPECTION
