29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about 8/28/2012, at the Oakwood location, the employer did not establish and implement a Lockout/Tagout program including specific written procedures, training and periodic inspections where employees provide service and maintenance activities to equipment at the facility including powered industrial trucks, fleet vehicles, roll up overhead doors, and dock plates.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire.    On or about 8/28/2012, in the maintenance shop area, oxygen and acetylene were stored together in a cage and not seperated by at least 20 feet.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.    On or about 8/28/2012, in the PIR area of the warehouse, the emergency eye wash located at the battery charging area was broken and had not been replaced while the battery charger was still in use.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.    Performance Food Group, Inc. was cited for the same or similar violation, 29 CFR 1910.151(c), in inspection 316494640, citation 1, item 1, issued on 6/4/2012 and became a final order 6/18/2012.
