29 CFR 1926.602(c)(1)(viii)(A): The safety platform was not firmly secured to the lifting carriage and/or forks. At this construction project a Skytrak 6042 fork lift truck was used to elevate employees in a shop made work platform without the platform being secured to the carriage or forks of the fork lift truck.

29 CFR 1926.759(a): All materials, equipment, and tools, which were not in use while aloft, were not secured against accidental displacement. On the roof under construction, 2 inch by 4 inch boards were wedged under the insulation blankets to keep the insulation rolls in place. The area directly below where the boards were wedged was not barricaded to protect employees on the ground floor from being struck.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Employees installing insulation blankets and metal decking on the roof of a new building under construction were not protected against falls. The employees did not use a personal fall arrest system or any other method to protect against falls up to 28 feet above the foundation slab.
